  • Stage 2: ≥140 / ≥90 mmHg

Managing diabetes doesn’t have to be overwhelming — we help you understand your numbers, medications, and what you want from your care. We follow the latest American Diabetes Association (ADA) guidelines, including:
 
  • Fasting blood sugar goal: 80–130 mg/dL
  • 2-hour post-meal goal: under 180 mg/dL
  • A1c goals are tailored to you, but if your A1c is above 9%, the ADA recommends starting insulin or a GLP-1 receptor agonist like Ozempic or Mounjaro. Why?
    Because it helps bring your blood sugar down quickly and safely, reducing the risk of long-term complications.
    That said, we always work with your comfort level — and never pressure anyone into treatments they’re not ready for.

We prioritize newer, safer medications
We typically avoid older drugs like sulfonylureas (glipizide, glimepiride), which:
  • Increase the risk of low blood sugar
  • May have negative effects on the heart
  • Are harder to manage with modern, flexible lifestyles
  • We'll help you track your sugars, adjust your treatment, and move toward better energy, fewer symptoms, and real peace of mind — all via secure telehealth throughout Alaska. Medicaid and private insurance accepted.
 
  • ⚠️ Hypoglycemia (low blood sugar) can happen — especially if you're on mealtime insulin or sulfonylureas.

    We’ll teach you the 15-15-15 rule:
    Eat 15g of fast-acting carbs, wait 15 minutes, and recheck.
    If your blood sugar is above 70, eat a balanced meal.
    If it remains below 70, repeat or call 911 if symptoms worsen.

Cholesterol treatment is often based on your estimated 10-year risk of a heart attack:

  • If your risk is above 5–7.5%, we may recommend starting a medication (like a statin) — especially if you have other risk factors.
  • We’ll walk you through what your numbers mean, not just hand you a printout.

Treatment options include:
  • Lifestyle and dietary changes (yes, even small ones help)
  • Medications when appropriate — like statins, ezetimibe, bempedoic acid, repatha
  • Ongoing lab monitoring and medication adjustments — coordinated fully by telehealth

🔍 What if your “bad” cholesterol (LDL) is really high?
  • If your LDL is over 160, that’s significantly elevated. In these cases, we may recommend a CT Coronary Artery Calcium (CAC) scan — a painless, low-radiation test that shows whether cholesterol is already building up in your arteries.
  • If your LDL is over 190, we consider the possibility of familial hypercholesterolemia (FH) — a genetic condition where the body can’t clear cholesterol normally. We’ll talk through next steps and make sure you get the care you need.
     

Worried About Statin Side Effects? We’ve Got Options.

Some people experience muscle aches, fatigue, or other side effects when taking statins — and that’s valid. We take statin intolerance seriously and work with you to find an approach that supports your heart without making you feel worse.

Here’s how we can manage statin sensitivity:
  • Switching to less lipophilic statins, like rosuvastatin or pitavastatin which may cause fewer side
  • Repletion of Vitamin D
  • Using lower or alternate-day dosing, especially for patients who only need modest LDL reduction
  • Adding non-statin medications like ezetimibe or bempedoic acid when statins aren’t tolerated
